Forms Marriott Electrical and Audio Visual Equipment Form - If you need electricity, download the form and follow the instructions. – Orders placed by Friday, 3/12/10 will receive a discounted rate. Temporary Food Permit Instructions Montgomery County Health Permit - A temporary health permit must be obtained by mail two (2) weeks in advance of the event date (Prior to Friday, March 12) or in person two (2) full business days before the event date (Prior to Wednesday, March 24). A late fee of $25.00 will apply if not obtained in advance. |
